Submission of the Project Progress Report for Fund Assistance for Houses of Worship

This service allows Houses of Worship that have received fund assistance to submit their project progress reports.
Softcopy of supporting documents as follows:
- 1 copy of the project implementation documents (Ex: percentage of project physical progress status etc.).
- 1 copy of the project payment documents (Ex: invoice, receipt/payment voucher, percentage of project financial status etc.)
- Latest pictures of project progress (in jpg, jpeg, gif or png format).
Submission of report can be made online by clicking button “Apply” below.
Houses of Worship of Other Religions that is registered with MSO/ROS/SSM/Charitable Trust and active, as well as owning or managing Houses of Worship or other religious facilities.
Project progress reports for status as of March & September every year must be submitted until the fund assistance received have been fully utilized or project completed.
Application is free of charge.
