1. Pictures related to the problems encountered or the needs (in jpg, jpeg, gif or png format).
2. 1 copy of the Bill of Quantity (if the project cost exceeds RM1.0 million).
3. 1 copy of the Quotation from the Supplier (if the project scope involves purchase of equipment/ equipment).
4. 1 copy of the front page of the bank book/ account statement of the Houses of Worship of Other Religions project certified by the Bank.
5. Passport size photo of the Project Management Committee (in jpg, jpeg, gif or png format).
6. Project Site/Space Approval Document:
   • 1 copy of the title document issued by the Government; or
   • Has obtained approval from the Ketua Masyarakat/Ketua Kaum, please attach 1 copy of the approval letter from the Ketua Masyarakat/Ketua Kaum.
   • 1 copy of Land Rights Claim Surrender Letter;
   • 1 copy of site/space use authorization letter (for religious facilities in public institutions). 
7. 1 copy of the building plan approval documents as stated in the special conditions of the title document (if relevant).
8. 1 copy of the work planning schedule & project monthly cash flow projections (if the project duration exceeds 1 year).
9. Recent pictures of project progress (if the project has started; in jpg, jpeg, gif or png format).

1. Applications that can be considered are for religious scope/purposes only and comply with the act/ ordinance/law/regulation currently in force.
2. Houses of Worship need to register their profiles at ORSEM first before able to make any application.
