2. Document on Training Hours (and/or relevant attachments) of relevant courses, seminars etc. (If issued with conditional renewal).
3. Copy of Business Registration (Business Names Ordinance (Chap. 64))/ Company Registration Certificate (Companies Act, 2016 (Section 17) (Act 777)).  Firms that are not registered in Sarawak should have office addresses in Sarawak.
4. Form I of the The Businesses, Professions and Trading Licensing Ordinance (Section 5,23 & 24(20)) (if applicable).
5. Copy of Form 49 of Companies Act, 2016 (Section 17) (Act 777) (if applicable).

• Period of study
• Area of study
• Verification from client (pls provide details on  company/contact person/ address, tel/fax/e-mail of client)

EIA Consultant Firms that have been registered with the NREB.

1. Applications that aren't complete and don't meet the criteria will be rejected.
2. The registration as EIA Consultant is valid for one year. The application for the renewal must be  submitted at least one month before the expiring date
3. A penalty of 20% will be charged for those renewal application made within three months after its expiry date.
4. The firm registration will be de-registered if they fail to comply with the following:

• The registration is not renewed after three (3) months of its expiry dates;
• The condition of Conditional Renewal.

5. For the firm that has been de-registered and wishes to be registered with the NREB, a penalty of Five Hundred Ringgit Malaysia (RM500.00) will be charged.
